Sorry for the delay in posting a summary, but my drive was delayed in
shipment, and I just got it hooked up this weekend.

I received numerous responses, many differing in not insubstantial
ways. Apparently the M2263SA was set up differently by different
vendors, in particular the number of sectors per track. Also everyone
seemed to be using different numbers for ncyl and pcyl. Not having
had experience with SunOS 4.[01], I didn't realize that a format.dat
entry is not necessary to initially format a disk; you really only
need the basic geometry numbers.
Here is the one I decided to use, compliments of jdd@db.toronto.edu:

disk_type = "Fujitsu M2263SA" : ctlr = MD21 \
        : ncyl = 1648 : acyl = 2 : pcyl = 1650 : nhead = 15 : nsect = 53 \
        : asect = 3 : trks_zone = 15 : atrks = 30 \
        : rpm = 3600 : bpt = 31296 \
        : cache = 0x11 \
        : skew = 2 : precomp = 1650

The only question that remains is whether the read-ahead cache should/must
be disabled under 4.1.1b. I didn't do that, and haven't encountered any
problems yet.

Further responses follow:
