Here's what I ended up using:

#
disk_type = "Seagate ST31200N" \
        : ctlr = SCSI : fmt_time = 4 \
        : trks_zone = 9 : asect = 5 : atrks = 5 \
        : ncyl = 2692 : acyl = 2 : pcyl = 2694: nhead = 9 : nsect = 85 \
        : rpm = 5400 : bpt = 36000
#
partition = "Seagate ST31200N" \
        : disk = "Seagate ST31200N" : ctlr = SCSI \
        : a = 0, 16830 : b = 22, 131580 : c = 0, 2059380 : g = 194, 286720 \
        : h = 569, 1624250

Several of the format.dat entries I received used different numbers for pcyl
(2700) and ncyl (2698) than above, which wouldn't work with the disk, although
these numbers are in agreement with the Seagate manual for this drive. The
Sun System Administration Procedures manual claims that the MD21 controller
requires several cylinders for its own internal use, which may explain this.
