As Dan Transue point out:
Try running boot -rp from the ok prompt. This will reconfigure (r) and
rebuild the path_to_inst file (p) as the machine comes up.

The first line is usually comment, so that's strange.

/etc/path_to_inst maps Openprom device names to device instances.
The importance of this file is that when you add a disk or controller,
all your otehr deives will keep their instance number.

The boot path_to_inst file can be recreated. Reboot with the -ar
option and speicfy /dev/null as path-to_inst file.
All other questions should be answered with the default answer.

You can remove the file and do a boot -r from the ok prompt. This file holds
configuration information that gets built when the os is installed. However, if
you change things like move sbus cards, etc, this file does not get changed. So
either move the file to another name ( if you're insecure about it ) or remove
it and reboot with the -r option. Another alternate to the boot -r you can also
touch /reconfigure. When the os sees that file it knows what to do with it. (
This way you can just reboot instead of halting the system first.)
