This is a follow summary from my original non-summary of 6 October 2004.

Thank you to

Tom Crummy and Gerard Henry who nailed my issue here
====================================================

Are these logging in via sshd? If so, patch 113273-08 causes it. The
patch has been withdrawn and I suggest you back out to 113273-07.

I've the same problem, there is a bad patch:
Bug ID: 5087792
Synopsis: patch 113273-08 breaks who and last from populating ip/hostname

Work Around:

patchrm 113273-08

Also, a special thank you to Ed Guenther who replied to me with multiple
suggestions.

Here is my original question
